The District has established a hotline to provide a process for reporting improper governmental activities. There are several ways that questions may be submitted.
Anyone wishing to make a report can dial 801-728-6890. Follow the voice prompts and you can leave an voice message that will be sent immediately to the proper personnel, who will reach out to you if you request a follow up call.
This Microsoft form, which can be filled out and submitted anonymously, or you can provide contact information. If you choose to submit information via this source there is not a way to submit documentation with the form. That would have to be done by email or regular mail. There are two forms, one is a general form which will be submitted to the District’s General Manager. The second form may be used if the complaint includes the District Manager and the form would be submitted to the Districts Board Chairperson.
